When traveling to/from Europe, does it cost more (or less) to fly IN to one
European city and OUT /back to the USA from another city?

We wish to fly from Florida (Tampa) to Milan, then from Rome back to Tampa.

Usually they just split the difference, so it's no big deal. Sometimes that
can't be done on special discount fares though.

Often it doesn't - I just bought a ticket on Lufthansa into Barcelona and
out of Madrid. It was actually $50 cheaper than Barcelona return. YMMV.

Not necessarily. Just book an open jaw trip. You will pay half of the
lowest available fare to Milan and half of the lowest available fare from
Rome.
